Treatments and Technology

Treatment depends on the underlying cause and severity of symptoms.

Treatment for male stress urinary incontinence:

  •  Hygienic measures: the first step is to recommend limiting water intake, especially in the hours prior to situations of greater risk, such as walking, shopping, sports, etc. In addition, it is recommended to urinate before being in situations that may trigger incontinence. In many cases these simple guidelines are sufficient in mild cases.
  • Pelvic floor rehabilitation (PFR): This is performed by activating and strengthening a series of pelvic muscles that help strengthen the urinary sphincter. There are many guidelines, but it is usually recommended tonic contraction of the urinary and anal sphincter for five seconds, repeating the exercise several times in the morning and evening, for 12 weeks. It is a very effective treatment for mild to moderate incontinence and is recommended after pelvic surgery or radiation therapy.
  • Medications: there are some medications to strengthen the urinary sphincter that can reduce the intensity of mild and moderate incontinence.

  • Surgery for male stress urinary incontinence: When conservative treatment is not effective, surgery may be considered. There are different types and devices that can be used in mild or moderate cases in order to support the male urethra. Like any surgery, it can lead to complications such as urinary retention, repeated urinary tract infections, infection or rejection of the surgical material, and in severe cases, the only alternative is the implantation of an artificial urinary sphincter. It is a kind of ring that surrounds the urethra, compressing it. The sphincter is activated by default, hugging the urethra and preventing urine leakage. When we want to urinate, the sphincter is temporarily deactivated, relieving the pressure on the urinary duct and allowing bladder emptying.

 

Treatment for male urinary incontinence of urgency:

  • Hygiene measures. The first step in the treatment of urge incontinence is voiding hygiene. In many cases, simply limiting water intake at certain times of the day or before certain activities can prevent urgency. In addition, we can recommend scheduled urination to prevent the bladder from overfilling and triggering urgency. Some foods, on the other hand, are harmful since they exacerbate bladder hyperactivity. Some of these are drinks with high caffeine content, carbonated or sweetened beverages, spicy foods and alcoholic beverages.
  • Rehabilitation. The first step is rehabilitation, which achieves great improvements in a short period of time, as well as the correction of dietary habits that may be aggravating the condition.
  • Medication. If rehabilitation is not sufficient, oral medication can be administered to "turn off" the urgency and urgency of the bladder. Some of the medications are: trospium, solifenacin, tolterodine or mirabegron. Depending on the severity and the presence of side effects, the use of one or more of these drugs will be necessary.
  • Surgery: Botox may be used to paralyze the bladder muscle. In an overactive bladder, the bladder muscle contracts more than usual. In this case Botox, as a muscle paralytic, works by paralyzing the muscle in a calculated dose. This helps to improve incontinence episodes and has a higher success rate than medication in many cases.Botox, apart from being an effective treatment for incontinence, allows men, on many occasions, to do without medication. The downside is that the effect of Botox is not permanent, since after 9-12 months the effect begins to wear off and it has to be reapplied. It is important to rule out the presence of obstruction before considering the application of botox to men, since this can lead to the appearance of urinary retention.

 

Treatment of mixed male urinary incontinence:

As a concept, the component that the patient expresses as predominant should be treated first. If this is not sufficient, we should treat the minority component. The treatment of this type of incontinence is very complex and usually requires highly specialized units.

 

Treatment of male urinary incontinence due to overflow:

The initial treatment is the placement of a bladder catheter to relieve pressure on the bladder. Subsequently, we carry out a study of the cause of the problem and the possible consequences it may have caused.
